What to Bring to
Your Appointment
To ensure a smooth billing process, please bring the following to your appointment:
- Your photo ID
- Your insurance card
- An updated medication list
- The name of your preferred pharmacy
- Any X-RAY, MRI, C T, or other reports detailing the reason for your pain
Referrals
We are accepting new patients for pain management by referral only. This referral can be from a primary care provider or another specialist who has treated you for pain in the past.
Referrals are not necessary for:
- Medical marijuana evaluations
- Painful cyst/lipoma removals
