We have an experienced team that is dedicated to helping patients with chronic pain.  We are always exploring new treatments that may be helpful to our patients.

if you are seeking medication management, have your primary care provider, or another specialists that has treated you for pain, fax a referral to our office at 833-992-0797.

It is important that we have a note from the previous provider regarding your pain problem and any pertinent imaging reports that may be available (such as MRI, CT, X-RAY). If it is not included in the referral, we will contact you and the provider's office to get additional information.

Once we have the medical records, we will verify your insurance and determine eligibility for services. Please see our list of accepted insurance. If we do not accept your insurance or you are a self pay patient, you will be notified of the fees before your appointment.

Your can expect to be in the office for between 1 to 2 hours for your initial appointment.
