Bring your photo ID, insurance card, updated medication list, and the name of your preferred pharmacy.
Any X-RAY, MRI, CT or other reports detailing the reason for your pain are also important.
You should plan on being in the clinic for your first appointment for around 2 hours.
Check your portal to see if you have any new forms to fill out before your next appointment to save time. Follow-up appointments are usually completed within an hour.

We are accepting new patients for pain management by referral only. It can be from a primary care provider or another specialist that has treated you for pain in the past.
Referrals are not necessary for medical marijuana evaluations, ketamine infusion consults, or painful cyst/lipoma removals.
If you have been scheduled to be sedated for your procedure or if you are having an epidural, you must have a driver present. If you are being sedated, you must fast from food and drink for 8 hours prior to your procedure. Patients that are taking blood thinners must talk to their provider about discontinuing medication prior to procedure day.
